Key Responsibilities of a Care Manager
1. Regular Home Visits
Conduct scheduled visits to members’ homes and ensure their wellbeing, comfort, and routine support.
2. Health Monitoring
Check and record basic vitals such as blood pressure, pulse, oxygen saturation, temperature, sugar (if required), and report abnormalities.
3. Emergency Response Support
Act promptly during emergencies by coordinating with family members, doctors, ambulance, or hospitals.
4. Medication & Appointment Assistance
Ensure timely medicine reminders, refill coordination, and assistance in doctor consultations / appointments.
5. Member Relationship Management
Build trust with elderly members through empathy, patience, companionship, and respectful communication.
6. Family Updates
Provide timely updates to children/family members, especially those staying away from parents.
7. Daily Need Assistance
Help coordinate groceries, utility tasks, bill payments, banking support, and small errands when required.
8. Vendor Coordination
Coordinate with nurses, attendants, physiotherapists, labs, pharmacies, and other service providers.
9. Documentation & Reporting
Maintain visit logs, task updates, digital records, and submit reports to management regularly.
10. Safety & Hygiene Checks
Observe home safety risks, hygiene conditions, fall hazards, and suggest preventive measures.
11. Emotional Support & Companionship
Offer meaningful conversation, emotional reassurance, and reduce loneliness among seniors.
12. Professional Conduct
Maintain punctuality, grooming, confidentiality, honesty, and company discipline at all times.
13. Service Upselling / Cross Support
Identify additional care needs and inform management about services that may benefit the member.
14. Travel Readiness
Be comfortable travelling across Kolkata for multiple home visits daily.
15. Crisis Handling with Calmness
Handle difficult family situations, medical stress, or emotional members with maturity and calmness.

Job Requirements:
The minimum qualification for this role is 12th Pass and 0 - 5 years of experience.
