Job Summary
The Hospital to Home Healthcare Nurse is responsible for delivering quality, patient-centered nursing care to individuals in hospital settings, patient homes, and corporate healthcare environments. The role focuses on ensuring patient safety, comfort, and improved health outcomes while maintaining dignity and independence.
Key Responsibilities
Assess patient health conditions and clinical care requirements.
Deliver skilled nursing care including wound dressing, IV therapy, medication administration, and vital sign monitoring.
Administer medications, injections, and vaccinations as prescribed by physicians.
Provide health education, guidance, and counselling to patients, family members, and caregivers.
Coordinate patient care with doctors, therapists, and other healthcare professionals.
Maintain accurate and timely documentation of patient assessments, treatment plans, and progress reports.
Monitor patient condition regularly and report any changes to the physician or clinical team.
Assist in rehabilitation and recovery support for patients when required.
Provide palliative and supportive care for patients with chronic or critical conditions.
Ensure patient confidentiality and adhere to hospital protocols, safety standards, and clinical guidelines.
Experience Requirements
Valid Nursing Registration (ANM / GNM).
0–2 years of experience in home healthcare, hospital care, or medical-surgical nursing preferred.
Good communication, interpersonal, and patient-care skills.
Ability to work independently and manage time effectively.
Work Environment
Deployment at hospitals (one-to-one patient care), patient homes, and corporate/industrial healthcare setups.
Work shifts include 12-hour duty (Day/Night) and 24-hour stay duty for one-to-one patient care at hospital or home settings.
Independent work environment with periodic supervision and clinical support.
