Job Title: Hospital to Home Health Care Nurse
Reports to: Clinical Manager/Director of Nursing
Location: Hospital, Patient Homes & Corporates
Qualification :- GNM Nursing
Job Summary:
Provide high-quality, patient-cantered nursing care to individuals in their homes, promoting independence, dignity, and optimal health outcomes.
Key Responsibilities:
1. Assess patient clinical requirements.
2. Provide skilled nursing services: wound care, IV therapy, medication management, vital sign monitoring.
3. Administer medications, injections, and vaccinations as prescribed.
4. Conduct health education and counselling for patients, families, and caregivers.
5. Coordinate care with healthcare providers, therapists, and community resources.
6. Document patient information, progress, and outcomes accurately.
7. Monitor patient condition, report changes to physicians and healthcare team.
9. Provide palliative care as needed.
10. Maintain patient confidentiality, adhere to healthcare guidelines.
Requirements:
1. Current RN licences.
2. Minimum 0-2 years of experience in home health, or medical-surgical nursing.
3. Excellent communication, interpersonal, and problem-solving abilities.
4. Ability to work independently, manage time effectively.
Physical Demands:
1. Travel to patient homes (transportation to and from will be provided by the organization itself).
2. Max age limit 35 yrs and min weight 40 kgs and above.
3. Exposure to infectious diseases, allergens.
Work Environment:
1. Hospital (1 to 1 patient), Patient homes, Corporates (Industrial shift).
2. 12 Hrs Shift duty (day/Nite) and 24 Hrs Stay duty (1 to 1 patient) at Hospital and Home.
3. Autonomous work environment, with periodic supervision.
