Responsibilities:
Patient Care & Coordination
• Handle new admissions and patient discharges.
• Coordinate with doctors and families during medical emergencies or patient deterioration.
• Provide weekly updates to families about patient activities and condition.
• Conduct daily patient check-ups and log vitals (TPR, BP), manually if required.
• Monitor the logbook for accuracy and completeness.
• Assist doctors during rounds and ensure prescription details are correctly filled.
• Update logbooks with any medication changes.
• Ensure all medications (including SOS/temporary) are administered correctly.
• Check medicine sheets and verify against actual administration.
Medical & Nursing Procedures
Oversee or perform:
• Bladder wash
• Wound dressing
• Bedsore check
• RT and catheter changes
• IV/IM administration
• Ensure proper documentation and creation of related procedure charts.
Inventory & Audit
• Cross-check medicine stock with logbook and excel sheet.
• Maintain monthly and weekly stock reports.
• Submit medicine purchase lists to Admin.
• Conduct regular audits:
• Logbook
• Medicines
• Surgical stock
• Patient hygiene
• Staff grooming
• Real-time diaper checks
Team & Shift Management
• Efficiently manage staff ROTA, especially in case of absenteeism.
• Train staff in nursing procedures and hygiene protocols.
• Conduct regular caregiver training.
• Monitor caregivers’ adherence to patient care goals and logbook updates.
Emergency & External Coordination
• Coordinate with hospitals, doctors, and therapists in emergencies.
• Ensure timely and appropriate patient treatment.
• Communicate with families and Admin regarding emergency updates
• Maintain proper documentation for emergencies and assist in death certification.
Sterilization & Equipment Management
Ensure:
• Gauze prepared by CGs is autoclaved with date labelled.
• Surgical equipment is sterilized weekly.
• First aid kits are stocked and ready.
• Nursing equipment maintenance and cleanliness is reported and tracked.
