Job Description

  • Shift: Roster-based shifts, including day and night shifts (24×7 nursing care)

    Roles:

    Provide hands-on nursing care at clients' homes as per prescribed care plans and doctors' instructions.

    • Monitor vital signs, assess patient health and maintain accurate nursing records.

    • Administer prescribed medications and perform authorised nursing procedures.

    • Provide wound care, skin care, pressure-injury prevention and mobility assistance.

    • Support elderly patients with limited mobility and age-related health conditions.

    • Care for patients with dementia, Alzheimer's, stroke, Parkinson's, diabetes, hypertension and other geriatric conditions.

    • Follow infection-control protocols and maintain a safe and hygienic care environment.

    • Identify changes in patients' physical or mental health and escalate emergencies promptly.

    • Communicate with family members, doctors and the coordination team regarding patient updates.

    • Conduct proper shift handovers and maintain continuity of care.
