We are looking for a compassionate and motivated Care Coach / Management Trainee to support the health, wellbeing and overall care management of senior citizens.

This opportunity is ideal for candidates from Hospital Management, Nursing, Pharmacy, Pharmaceutical or Healthcare backgrounds who want to build a career in senior care and care coordination.

🔹 Key Responsibilities

• Assist in developing and implementing individualized care plans for senior citizens.

• Assess physical, emotional and social needs and coordinate appropriate support.

• Coordinate with doctors, healthcare providers, family members and caregivers.

• Educate clients and families regarding medication management and health maintenance.

• Monitor client progress and update care plans as required.

• Maintain proper documentation, reports and client records.

• Support administrative and operational activities related to senior care.

• Participate in training programs and workshops.

• Stay updated with senior-care practices and healthcare trends.

🔹 Candidate Profile

• Bachelor’s degree in Healthcare Management, Nursing, Pharmacy or a related field.

• Master’s degree will be an added advantage.
