Home Care Nursing

Conduct initial and ongoing physical assessments
- Monitor vital signs (blood pressure, pulse, temperature, etc.)
- Observe and document changes in health status

2. Medication Management- Administer prescribed medications (oral, injectable, IV)
- Educate patient/family on proper medication use
- Monitor for side effects or reactions

3. Wound Care & Medical Procedures- Clean and dress wounds
- Change catheters, colostomy bags, tracheostomy tubes
- Administer IV therapy or infusions
- Post-operative care

4. Care Planning- Develop, implement, and adjust personalized care plans
- Coordinate with doctors, physical therapists, and other providers
- Document and report progress or concerns

5. Patient & Family Education- Teach disease management (e.g., diabetes, hypertension)
- Guide caregivers on proper care techniques
- Offer emotional support and guidance

6. Equipment Management- Set up and maintain medical equipment (oxygen, feeding tubes, etc.)
- Train families on equipment use
