Role Summary

The Home Care Nurse is responsible for providing professional nursing care to patients at home as per physician orders and clinical protocols. The role includes administering medications, monitoring patient health, assisting with daily living activities, maintaining medical records, and coordinating with doctors and caregivers to ensure holistic patient care.

Key Responsibilities

Patient Care

• Provide nursing care to patients at home as per the treatment plan.

• Administer oral and IV medications, injections, wound dressings, and catheter care.

• Monitor vital signs (BP, pulse, temperature, SpO₂, etc.) and report abnormalities.

• Assist patients with mobility, hygiene, feeding, and daily living activities.

• Manage medical equipment such as oxygen concentrators, suction machines, and infusion pumps.

Clinical Support & Documentation

• Maintain accurate and up-to-date patient care records.

• Follow infection control protocols and ensure patient safety.

• Observe, assess, and document changes in patient condition; report to physician/nurse supervisor.

• Support doctors during home visits or teleconsultations.

Coordination & Communication

• Coordinate with family members and caregivers to educate them on patient care routines.

• Communicate effectively with the medical and home care team.

• Ensure continuity of care by updating the nursing supervisor on daily activities.

Emergency Management

• Identify and manage medical emergencies appropriately.

• Administer first aid and escalate serious conditions promptly.

Compliance & Ethics

• Maintain confidentiality of patient information.

• Follow all clinical, ethical, and professional nursing standards and policies.

Qualifications & Experience

• Education: GNM / B.Sc. Nursing / ANM (Registered Nurse)

• Experience: 0–5 years in hospital or home care setup (freshers may be trained).

• Valid Nursing Registration Certificate (State Nursing Council).

• Basic Life Support (BLS) certification preferred.

Work Environment & Conditions

• Home visit–based role (travel to patient’s home).

• Flexible working hours / shift-based assignments as per patient requirement.
