Lead, supervise, and coordinate a team of home healthcare nurses, ensuring
high standards of patient care.
Create and manage nurse schedules to ensure timely and adequate coverage
for patient visits.
Train, mentor, and support nursing staff to deliver care following best
practices and safety protocols.
Conduct performance evaluations, identifying areas for improvement and
development within the team.
Clinical Oversight:
Oversee the clinical care provided by the nursing team, ensuring it meets
regulatory and quality standards.
Review and approve patient care plans, ensuring they are tailored to each
patient’s specific needs and conditions.
Provide clinical guidance and intervention in complex or critical cases when
needed.
Ensure all nurses are trained and compliant with infection control, medication
administration, and home safety procedures.
Monitor patient outcomes and adjust care plans to improve the quality of care.
Operational Management:
Manage the day-to-day operations of home healthcare services, ensuring
smooth workflow and high productivity.
Maintain and update patient records, schedules, and care plans using data
management systems.
Ensure compliance with healthcare regulations, including patient privacy and
safety requirements.
Oversee inventory and ensure nursing staff have the necessary medical
supplies and equipment for in-home care.
Technical Skills and Data Management:
Use Microsoft Excel to manage and analyze data such as patient care
metrics, nurse schedules, staffing levels, and performance KPIs.
Create reports and dashboards using Excel functions (e.g., pivot tables,
VLOOKUP, charts) to support management in decision-making.
Track patient outcomes, staff productivity, and overall service performance
through data analysis, identifying trends for continuous improvement.
Coordinate the use of electronic health records (EHR) systems and other
software tools to streamline operations and patient care documentation.
Quality Assurance and Compliance:
Ensure that home healthcare services adhere to all state and federal
healthcare regulations.
Implement and monitor quality assurance programs to improve patient
outcomes and reduce risk.
Participate in audits, inspections, and reviews to ensure compliance with
accreditation standards.
Foster a culture of continuous improvement by encouraging staff to participate
in ongoing training and professional development.
Communication and Coordination:
Act as the primary point of contact for patients, families, and caregivers,
ensuring clear communication and coordination of care.
Hold regular team meetings to discuss patient cases, operational updates,
and performance reviews.
Collaborate with external partners and providers (e.g., physicians,
pharmacies, therapists) to ensure comprehensive care delivery for patients.
Report key performance metrics and operational updates to senior
management.
