Narayana Health is in the process of setting up a strategic team tasked with bringing healthcare closer to patients. The initiative involves setting up primary and secondary care services that are easily accessible to patients and are supported by hyper-responsive processes built to keep patient needs and satisfaction at the center of focus. The initiative aims to bridge the gap between Narayana Health’s in hospital offering and patient’s everyday healthcare needs and provide patients access to a continuum of care tailored to their own specific healthcare needs.
The primary role of the Patient Care Coordinator is to deliver Care by coordinating between patients on the provider’s daily schedule and by proactive interventions.
• Will act as a conduit for the transmission of information between providers and Customers. • Will coordinate with Medical and Support services for all Customers.
• Will facilitate information flow and referral services for customers to address their medical and psychosocial needs.
