Conduct initial and ongoing assessments of the patient's physical, emotional, and mental health.
Monitor changes in condition and respond accordingly.
Evaluate home safety and make recommendations for modifications if needed.
Develop individualized care plans in collaboration with physicians, patients, families, and caregivers.
Coordinate with other healthcare professionals such as physical therapists, occupational therapists, and social workers.
Adjust care plans based on patient needs and progress.
Administer prescribed medications and injections.
Monitor for side effects and interactions.
Educate patients and caregivers about proper medication use.
Clean and dress wounds.
Monitor for signs of infection or delayed healing.
Provide skin care for immobile patients to prevent bedsores.
Educate patients and families on disease processes, treatments, nutrition, and hygiene.
Provide emotional support and counseling when needed.
Maintain accurate and timely nursing notes and records.
Report changes in patient status to physicians and other care team members.
Oversee Nursing Assistants, and other support staff.
Ensure that delegated tasks are carried out appropriately.
Recognize and respond to medical emergencies.
Provide CPR or other immediate interventions until emergency services arrive.
