Duties and Responsibilities
1. Alignment with Nursing Protocols
Align with the nursing care and goals of KSC as per protocols, SOPs, and policies.
2. Orientation and Floor Routine Management
Get oriented to floor routines, patient numbers, risk categories, and basic nursing procedures.
Maintain and update equipment checklists and manage inventory through HMS.
3. Coordination and Communication
Coordinate with senior staff nurses regarding admissions, discharges, transfers, referrals, and lab diagnostics.
Communicate significant changes in patient conditions to appropriate healthcare team members promptly.
4. Comprehensive Nursing Assessment
Complete comprehensive nursing assessments of all patients within the TAT.
Document nursing assessments and care plans in patient files.
5. Medical Records and Documentation
Manage MRD files (open and closed).
Maintain clear and concise documentation of patient assessments, interventions, and progress.
Ensure proper documentation of verbal orders given during emergencies by doctors.
6. Direct Patient Care
Provide direct nursing care, including:
Medication reconciliation and administration.
Wound care and chronic disease management.
Palliative and end-of-life care using best practices.
Assist with personal hygiene tasks (bathing, dressing, grooming, and toileting).
Provide mobility assistance, including transfers, walking, and using assistive devices.
7. Medication Administration and Monitoring
Accurately administer medications as prescribed, observing actions and monitoring for potential side effects.
Identify 2 identifiers before administering medications.
Administer high-alert medications only after a senior nurse's verification, following the medication administration rights.
Report any medication errors immediately to the Nurse Manager and CMO.
8. Ward Rounds and Assistance
Accompany the CMO during floor/ward rounds, prepare for rounds, and assist with procedures as needed.
Ensure all investigations are carried out as advised and reports are made available promptly.
9. Inventory and Supplies Management
Raise indents and ensure stock availability.
Inform the Nurse Manager immediately about any shortages for timely resolution.
10. Patient-Centered Care
Demonstrate empathy, patience, and sensitivity while caring for geriatric patients.
Ensure compassionate, person-centered care that preserves patient dignity, autonomy, and quality of life.
11. Safety and Infection Control
Follow all safety measures for patient care and self-protection.
Implement fall prevention strategies 24/7.
Follow infection control practices and proper segregation and disposal of biomedical waste.
12. Team Collaboration
Collaborate with other healthcare team members, including doctors, therapists, and family members, to ensure coordinated care.
Ensure proper take-over and hand-over during each shift.
13. Discipline and Training
Maintain strict discipline and decorum at all times.
Attend in-service programs and training sessions to enhance knowledge and skills.
14. Incident Reporting
Report incidents promptly to the Nurse Manager with Root Cause Analysis (RCA) and Corrective and Preventive Action (CAPA) using the incident reporting form.
15. Quality Improvement and Patient Rights
Update QI data on time.
Be aware of and uphold patient rights while maintaining confidentiality.
16. Transfer Formalities
Complete transfer formalities as per the transfer policy.
17. Oversee the entire nursing operations during the respective shift
