The Nurse in the Chronic Patient Care team at Cloudphysician's Clinical Capability

Center (CCC) is integral to delivering exceptional healthcare to patients with chronic

conditions. This role serves as a liaison between patients and healthcare providers,

ensuring effective communication, accurate information exchange, and coordinated

care to enhance patient outcomes. The nurse will address clinical queries, manage

patient concerns, and provide ongoing support to guarantee optimal care.

Key Responsibilities:

Patient Care:

● Ensure timely and accurate placement of medication orders in the pharmacy,

based on precise dosage calculations, strength, and quantity requirements, in

accordance with the provider's prescription approval.

● Collaborate with healthcare teams, including US-based doctors and nurses, to

assess and ensure continuity of care.

Clinical Communication & Collaboration:

● Maintain seamless communication with US providers, facilitating the smooth

transfer of patient information and ensuring the coordination of care.

Nursing Documentation & Compliance:

● Adhere to US nursing standards, protocols, and regulations, ensuring

compliance with HIPAA and other legal frameworks for patient confidentiality

and privacy.
