Position Title: Home Care Nurse

Job Summary:

We are seeking compassionate and skilled nurses (B.Sc. Nursing / GNM) to provide high-quality care to patients in their homes. The Home Care Nurse will assess, plan, and implement nursing care plans, ensuring patients' comfort and recovery. This role demands professionalism, excellent communication, and the ability to work independently.

Key Responsibilities:

Patient Care:

Provide personalized care based on patients' medical needs, including wound care, administering medications, and monitoring vital signs.

Assist patients with daily activities, including hygiene, mobility, and nutrition.

Educate patients and families on managing medical conditions and using medical equipment.

Clinical Support:

Perform health assessments and document patient progress.

Administer injections, IV therapy, and other nursing interventions as prescribed by physicians.

Handle emergency situations with prompt and effective measures.

Documentation:

Maintain accurate and detailed patient records, including treatment plans and progress notes.

Update healthcare teams regularly regarding patient condition and care plans.

Collaboration:

Work closely with physicians, therapists, and other healthcare professionals to ensure coordinated care.

Provide emotional and psychological support to patients and their families.

Compliance and Safety:

Adhere to all health and safety standards, infection control protocols, and home care policies.

Ensure proper handling of medical equipment and medications.

Skills:

• Strong clinical and interpersonal skills.

• Ability to work independently and manage time effectively.

• Familiarity with electronic health records (EHR) and documentation.
